How to register

To register at the practice you will need to download a GMS1 and our new patient registration form below.

These will need to be filled in and brought into the closest branch to you.



In addition you will need to bring the following identification:

1.YOUR PASSPORT OR PHOTOGRAPHIC DRIVING LICENCE

If you have neither of these documents we will require your birth certificate.  Please note, for new babies, we will require a birth certificate and NHS number, which will be given to the mother before discharge from the maternity/child helth department.
           
AND


2.TWO RECENT UTILITY BILLS
These may include council tax, gas, electricity or water.  If you are renting a property please provide a copy of your tenancy agreement.

These documents will be copied and retained for as long as necessary in your medical records.  Failure to produce these documents may result in your registration being refused.

Once your forms are complete, the registration process usually takes around 5 days after which you are required to book with the practice nurse. Once the medical has been done you are then free to use our services.

  • Summary Care Record

    A Summary Care Record is an electronic record which contains information about the medicines you take, allergies you suffer from and any bad reactions to medicines you have had.

    Having this information stored in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed.
